Axis Health System

Axis Health System is a private, not-for-profit healthcare organization serving the 5 counties of southwest Colorado. Since 2006, Axis has been transforming to an integrated health system that seamlessly blends mental health, substance use, primary health, and preventative care. Axis operates two federally-designated Community Health Centers (CHC) – La Plata Integrated Healthcare in Durango, and Cortez Integrated Healthcare – as fully integrated primary care clinics. Axis also serves as the medical sponsor for the two District 9-R School-Based Health Centers (at Durango High and Florida Mesa Elementary Schools). Two other Durango locations, Axis's.Crossroads at Grandview and Columbine Center, respectively, serve a critical regional need providing acute treatment, detox, and regional crisis care and, along with the Pagosa Springs Clinic, outpatient behavioral healthcare services. Axis Health System has been providing healthcare services to Archuleta, Dolores, La Plata, Montezuma, and San Juan counties for more than 50 years.
