THE UNITED LEUKODYSTROPHY FOUNDATION INC

The United Leukodystrophy Foundation, Inc. was incorporated on June 15, 1982 under the State of Indiana Not-For-Profit Corporation Act of 1971. The Foundation is exempt from Federal and State income taxes under Internal Revenue Code 501(c)(3) and similar provisions of Sate income tax law by Internal Revenue exemption letter dated March 31, 1983. The Foundation was organized: - To provide leukodystrophy patients and their families with information about leukodystrophy and to assist them identifying sources of medical care, social services and genetic counseling. - To establish and coordinate a communication network among affected families. - To increase public awareness about leukodystrophy and to act as a source of information for health care providers. - To promote and support research on the causes, treatment and prevention of the leukodystrophies.
